LIABILITY WAIVER FOR SEA STAR STUDIOS 

COVID-19 ASSUMPTION OF THE RISKS AND WAIVER OF LIABILITY. As a willing participant in classes at Sea Star Studios, my family and I agree to the following:
ASSUMPTION OF THE RISKS:
I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19), may exist in the community, including at Sea Star Studios, and present an inherent risk that cannot be eliminated. I acknowledge that, although Sea Star Studios is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people in our classes (including our teachers, and third parties), it is ultimately my own responsibility to ensure that I take appropriate actions to safeguard myself and my child/children, including all adults, when arriving to, participating in, or departing from creative arts classes. While using the Playgym I agree to accompany my child at all times and use common sense social distancing guidelines. I understand and agree that as a registered family with Sea Star Studios, I am accepting and voluntarily assuming the risk that my child or I may be exposed and become ill as a result of COVID-19 and other diseases.

WAIVER OF LIABILITY: I, on behalf of myself and my children, hereby voluntarily release, forever discharge, and covenant not to sue Sea Star Studios and its owners, managers, contractors, members, employees, agents, and volunteers, or the location owner (“Released Parties”) for any claims that may arise out of or relate in any way to my or my child’s exposure to any communicable disease, including but not limited to the novel coronavirus (COVID-19) or any other accident that may cause harm. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties.

I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. BY ENROLLING IN CLASSES AT SEA STAR STUDIOS, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT.

Health Assessment Before you leave home: Please do a quick health assessment and stay home if you or anyone living in your home is showing any of these symptoms: fever, cough, shortness of breath, difficulty breathing, other respiratory symptoms, or any two of these symptoms: chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. Please stay home and contact us at [email protected] if you or anyone you've been in close contact with has been exposed to COVID-19.  If there is a known in-class exposure in the studio we will contact the Mendocino County Health Department and follow their current guidelines.

GENERAL TERMS AND CONDITIONS FOR Music Together® AND CREATIVE ARTS CLASSES 

For Music Together, there is a maximum 12 families per class. Babies under 8 months are free, however, all children attending class must be enrolled. Please choose the sibling under 8 months option when enrolling your baby under 8 months. All other creative arts classes have age ranges and enrollment limits as specified on the enrollment page.

Make-up classes for Music Together: For your convenience, we offer 2 makeup classes per Music Together session. Makeup classes are scheduled on a first come, first served, honor system basis, and must be used within the current session. Makeup classes can be scheduled using our online make-up scheduler. Please be considerate of your fellow students: If you cannot attend a scheduled makeup class, please cancel as soon as possible so someone else can use that spot. We are not currently able to provide makeups for other creative arts classes, but contact [email protected] if you have to miss a class.

Refund Policy: Once confirmed in a Music Together class, a $25 processing fee will be deducted for withdrawals if cancelled before the session begins. No refunds are given after the current session begins. Opened Music Together materials cannot be refunded/returned and must be purchased at a cost of $45. All other creative arts classes will incur a $15 processing fee for withdrawals if cancelled before the session begins. No refunds are given after a session begins.

Safety: Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained during at Sea Star Studios resulting from normal class activity or any other activity conducted by families before, during, or after class time.
